By using WholeChart you agree to these terms. They are short because the product's role is deliberately narrow: it organizes health information you provide. It does not practice medicine.
Not medical advice — ever
- WholeChart is not a medical device, not a diagnostic tool, and not a substitute for a clinician. Nothing it produces — including AI-generated discussion topics, “worth asking your doctor about” items, summaries, or the rules-based red-flag layer — is a diagnosis, treatment recommendation, or medical advice.
- AI output is generated from the information you entered and can be incomplete or wrong. It is labeled in the app and is intended only as preparation material for conversations with licensed clinicians.
- The red-flag layer is a simple pattern check. It cannot determine whether something is an emergency. If you may be experiencing a medical emergency, call your local emergency number immediately — do not rely on this app.
Your responsibilities
- You are responsible for the accuracy of what you enter and for verifying extracted values against the original documents. Values read from a document are saved as unverified and stay marked that way — and are left out of the report's lab trends — until you confirm them, but they are part of your record from the moment they are extracted.
- Recording other people (including clinicians) may require their consent depending on your location. Obtain consent before recording appointments; you are responsible for complying with local law.
- Keep your credentials secure. We strongly recommend enabling two-factor authentication.
- Use the service only for yourself and only for lawful purposes.
Your data
Your record remains yours. Our handling of it is described in the privacy policy, which is part of these terms. You can export your record as a PDF or image report — including a full-record appendix — download your original files and recordings from within the app, and delete your account at any time.
Service provided “as is”
We work hard to keep the service reliable and your data safe, but the service is provided “as is,” without warranties of any kind. To the maximum extent permitted by law, our liability arising from your use of the service is limited to the amount you paid for it in the twelve months before the claim.
Changes
We may update these terms; material changes will be announced in the app before they take effect. Continued use after that constitutes acceptance.
